Title Appointments
Document requirements vary depending on the type of title transaction. Click on a section below to expand information.
The seller must sign the title over to you and complete the “Transfer of Title by Seller” section on the Florida title, including the sale date, sale price and current odometer reading and date.
Bring:
- The title;
- Proof of Florida insurance on the vehicle; and
- Valid driver’s license/ state ID with you.
All new listed owners must be present to sign the title application.
The title certificate must be signed over to you by the seller and include the sale date, sale price and current odometer reading and date.
Bring:
- The vehicle for a VIN verification;
- Title;
- Proof of Florida insurance on the vehicle; and
- Valid driver’s license/ state ID with you.

If you own the car, bring:
- The title;
- Vehicle for a VIN verification;
- Proof of Florida insurance on the vehicle; and

If the car is financed, bring:
- The lienholder (finance company) information;
- Current vehicle registration;

Driver's License/ ID Appointments
Document requirements vary depending on whether or not the customer is currently REAL ID compliant in Florida, which is indicated by a star in the upper right-hand corner of the Florida driver's license or ID card, and their immigration/ residency status. I Need To
Get My Florida Learner's License
You must bring documents that validate your identity, Social Security Number (SSN) and Orange County residential address. For most customers, those documents are 1) your birth certificate or passport and 2) your social security card and 3) two proofs of Orange County residential address from a parent or guardian.

If Your Name Has Changed
If you have legally changed your name by marriage, divorce or court order, you must submit an original or certified U.S. marriage certificate(s), U.S. divorce document(s) or other court order(s). Copies are not accepted unless certified by the issuing authority.
Marriage certificates must be recognized by the state of Florida. Generally, marriage certificates issued in Florida or another state in accordance with the respective laws of those states are recognized by the State of Florida. U.S. Citizens & Permanent Residents
An original or certified copy of one of the following documents:
- U.S. birth certificate (hospital certificates are not accepted)
- Valid U.S. passport or passport card
- Consular Report of Birth Abroad
- Certificate of Naturalization, form N-550 or form N-570
- Certificate of Citizenship, form N-560 or form N-561
Notice to Puerto Rican Customers
- Puerto Rican customers must present a birth certificate that was issued after July 1, 2010
Immigrants
An original or certified copy of one of the following documents:
- Valid alien registration receipt card (green card, form I-551)
- I-551 stamp in passport or on I-94
- Immigration judge’s order, containing the customer’s A-number (admissions number), granting asylum
- I-797, containing the customer’s A-number, stating the customer has been granted asylum
- I-797, or another form from the United States Citizenship and Immigration Services (USCIS), with the customer’s A-number, stating the customer’s application for refugee status is approved
- Valid passport
Nonimmigrants
An original or certified copy of one of the following documents:
- Valid employment authorization card issued by the U.S. Department of Homeland Security (DHS) form I-688B or I-766
- Non-expired proof of nonimmigrant classification provided by DHS form I-94 with required supporting attachment(s). If a customer is unsure of the required documentation, they should bring all United States Citizenship and Immigration Services (USCIS) documentation with them. I-94s must be accompanied by a valid passport with a date of entry stamp. Certain classifications require additional documentation. Some examples are F-1 and M-1 classification must be accompanied by an I-20; J-1 or J-2 designation must be accompanied by a DS2019; and Refugee, asylee and parolee classifications must be accompanied by additional documentation
- I-571 travel document/refugee travel permit
- I-512 parole letter accepted
- Immigration judge’s order granting asylum or cancellation of removal
- Valid passport
Notice
Please be advised:
- Documents must be valid for more than 30 days from the date of issuance.
- Non-U.S. citizens applying for an original driver license will be issued a 60-day temporary paper permit without a photo and a receipt.
- Non-U.S. citizens applying for an ID card will be issued a receipt.
- Upon identity and legal status verification, a driver license or ID card will be issued within 60 days and mailed to the address on the driver record.
- The driver license or ID card will be issued for the period of time specified on the USCIS document, up to a maximum of four years.
- If lawful status or identification is not verified, a denial of issuance letter will be mailed to the customer.
Social Security card (copies are not accepted)
Option 1
Provide two documents from a parent, step-parent or legal guardian from the list below and a Parental Consent form from the person whose name is on the residential address documents. The individual whose name is on the documents and Parental Consent form must appear with you in person and sign the form in the presence of our personnel or they may sign the form in the presence of a notary public.
Option 2
Provide two documents from a parent, step-parent, legal guardian or other person with whom you reside from the list below and document(s) demonstrating family relationship, such as a birth certificate, marriage certificate, military dependent identification card and adoption record.
Option 3
If you're unable to provide documents to establish family relationship, you may provide two documents from a parent, step-parent, legal guardian or other person with whom you reside from the list below and a Certification of Address form from the person whose name is on the residential address documents. The individual whose name is on the documents and Certification of Address form must appear with you in person and sign the form in the presence of our personnel or they may sign the document in the presence of a notary public.

- Professional license issued by a government agency in the U.S.
- W-2 form or 1099 form
- Form DS2019, Certificate of Eligibility for Exchange Visitor (J-1) status
- A letter from a homeless shelter, transitional service provider, or half-way house verifying that the customer resides at the shelter address
- Mail from financial institutions; including checking, savings, credit card statements or investment account statements
- Mail from Federal, State, County or City government agencies (including city and county agencies)
- Personal mail that does not list the recipient as “occupant”. Example: Magazines, journals, etc.
- Florida Department of Law Enforcement (FDLE) Registration form completed by the local sheriff’s department or Department of

Customers who do not have any of the above documents in their name may provide two proofs of residential address from a parent, step-parent, legal guardian or other person with whom they reside and a Certification of Address form.
Change My Name on My Florida Driver's License or ID
Follow these steps to change your name on your driver’s license or ID.
Immigrants & Nonimmigrants
Customers who have legally changed their name by marriage, divorce or court order must also update their USCIS documents to reflect their name change before they can update their name on their driver's license or ID.
Name changes must first be made with the Federal Social Security Administration in order to have your name updated on your driver’s license or ID.
To change your name with Social Security, you must present documents proving your legal name change and identity. If you are a U.S. citizen, you must also present documents proving your U.S. citizenship. Only original documents or copies certified by the issuing agency are accepted.
To prove your legal name change, you must show one of the following documents.
- Marriage document;
- Divorce decree;
- Certificate of Naturalization showing a new name; or
- Court order for a name change.
To prove your identity, you must show one unexpired document showing your name, identifying information, and photograph, such as:
- State-issued U.S. driver’s license;
- State-issued ID; or
- Valid U.S. Passport.
To prove your U.S. citizenship, you must show one of the following:
- U.S. birth certificate;
- U.S. Consular Report of Birth Abroad;
- Valid U.S. Passport;
- Certificate of Naturalization; or
- Certificate of Citizenship.
Please allow 24 to 48 hours after completing the name change with the Social Security Administration.
REAL ID compliance is indicated by a star on the upper right-hand corner of your Florida driver's license or ID.
1. Are You Real ID Compliant?
- Yes, I have a star: If you are currently REAL ID compliant, you will only need to provide your name change document(s) (see below).
- No, I do not have a star: If you are not currently REAL ID compliant, you must bring documents that validate your identity, Social Security Number (SSN) and Orange County residential address in addition to your name change document(s). For most customers, those documents are 1) your birth certificate or passport and 2) your social security card and 3) two proofs of Orange County residential address.
2. Name Change Documents

Please Note:
- Only a birth certificate issued by a county health department or the CDC Bureau of Vital Statistics will be accepted. Hospital birth certificates are not considered a certified document and will not be accepted.
- Please come prepared to present one of the above listed identification documents as proof of citizenship or legal presence.
- When applicable, marriage certificates, court orders or divorce decrees must be provided to link the name on the primary identification to the name on the driver license or ID card.
- If a valid U.S. passport has the customer’s current name, they are not required to present additional name change documents.

Written (Learner's License) & Road Driving Test Appointments
Written Test
To obtain a learner's license, customers must be at least 15-years-old, complete the Traffic Law and Substance Abuse Education Course, pass the Road Signs and Road Rules Test (aka written test) and pass the Vision and Hearing Test.
Driving Test
To obtain an Intermediate Driver’s License, customers must be at least 16-years-old and have held their learner’s license for at least one year or until their 18th birthday with no traffic convictions and pass the Road Test.
Tag Renewal Appointments
Please bring your:
- Previous registration or registration renewal notice;
- Florida driver's license; and
- Current proof of Florida Property Damage and Personal Injury Protection insurance issued by a Florida agent or broker.
Please Note
Various suspensions and violations, such as parking and toll violations, may place a hold on your driver’s license and registration.
Disabled Parking Placard Appointments
Please download, complete and bring the Application for Disabled Parking Permit. There is no charge for a four-year Disabled Parking Permit; the cost for a Temporary Disabled Parking Permit is $15.
